Health Care Spending Paper Essay

This paper will include: the current health care expenditures whether spending is too much or not enough, where the nation should add or cut, how the public’s health care needs are paid and provide a forecast for: the future economic needs, why these needs must be addressed, how I envision these needs will be financed and conclusion. Current Health Expenditures Health care costs have been rising for several years. Expenditures in the United States on health care surpassed $2.3 trillion in 2008, more than three times the $714 billion spent in 1990, and over eight times the $253 billion spent in 1980. Stemming this growth has become a major policy priority, as the government, employers, and consumers increasingly struggle to keep up with health care cost. Hospital spending, the largest share of overall health care spending, is a major driver of increased costs The current national expenditure have tripled over the last decade especially in hospital care, physician/clinical services and drug ( Kaiser, 2010). The United States is at an all time high in health care. In the health care system there are some factors that contribute to this increase. It is getting to the point that more and more Americans cannot afford health care, because of the cost of health care is getting harder to maintain for their families. Whether Spending is too much or not enough It is difficult to pin point one exact factor why spending is escalating. The chart below states that hospital care, physician/clinical services and prescription drugs are where the most spending is taking place (Kaiser, 2010). This shows we are spending too much, especially in these top three categories listed. When health care cost is steadily increasing, so we are spending a great deal in the list above. National Health Expenditures, 2008 [pic] Source: Centers for Medicare and Medicaid Services, Office of the Actuary, National Health Statistics Group Where the Nation Should Add or Cut Medicare and Medicaid account for a significant share of health care spending (Kaiser, 2010) and also according to the above chart we could cut in the hospital care, physician/clinical services and prescription drugs. This is why we need to cut as far as the nation is concerned because these government programs Medicare and Medicaid are funding health cost at a steady growth. We have to work on controlling this program before it controls us. If we do not start cutting from somewhere we will be in a serious deficit for health care in the United States. Then we need to cut prescription drugs cost, so it can be affordable to everyone, so the government needs to intercede the pharmaceutical companies closely to manage this expense. How the Public’s Health Care needs are Paid Although Americans benefit from many of the investments in health care, the recent rapid cost growth, coupled with an overall economic slowdown and rising federal deficit, is placing great strains on the systems used to finance health care, including private employer-sponsored health insurance  coverage and public insurance programs such as Medicare and Medicaid ( Kaiser, 2010 ). The public’s health care payments are paid by Medicaid and Medicare if eligible for this government health program and other private health insurance companies, for example: Blue cross and Blue Shield, Aetna, Cigna and United Health Care are just a few the public health care is paid. There are some people who pay out of pocket, whether they can afford it or not. Forecast: the Future Economic needs Scott Donahue, vice president of Triple Tree healthcare advisory group, agrees, pointing to statistics such as those found in the 2009 PricewaterhouseCoopers Health Research Institute study that revealed nearly one half of the $2.2 trillion the United States spends each year on healthcare is wasted. â€Å"While it is impossible to point to any single breakdown contributing to the waste, it is clear from the study’s findings that this is an industry that has significantly underutilized technology to improve efficiencies,† he says, adding that cloud computing could help mend a healthcare system that is stuck in the inefficiencies and limitations found in older legacy systems (Chavis, 2011). There needs to be an investment in our technology system for health care for example: electronic medical records (EMR) to better serve the patients and technology that is being required to give quality service and modify the provider’s wages, since in the hospital this is the biggest cost. This is a start in the direction to accommodate the Americans who cannot afford health care. These small changes can contribute to a clear forecast in health care. Forecast: why these needs must be address This issue needs to be address because health care cost is soaring and if we do not intercept and make some real changes the American people will not afford medical services. The health provider’s and facilities are making big money and they need to work with the people to help with this expense we have made. The government can help by fine tuning the Medicare and Medicaid  program, so people can benefit fairly from it. We need to get proactive in prevention, so we our bodies will stay healthy for a long life. Forecast: how I envision these needs will be financed I envision a new administration making some major changes to make health care affordable for all Americans. The private insurance companies will need to work on some alternative health care package to their consumers for health care cost. The government in Washington to stop thinking of them and focus on health care cost, so all can manage health expense and get continued quality care also. Conclusion The bottom line we need to concentrate on cutting cost of health care and quality care, which we all deserve as human beings. There is not just one factor that contributes to the health expenditure but several pieces to this puzzle that makes this a major health cost issue. We need to find ways to decrease spending too much, so people can benefit from their health plan. The nation should consider cutting hospital care, clinical services and prescription drugs, since these are the top expenses for health care. This would definitely clear a path for reduced cost. The public can barely afford to pay premiums to their private health insurance companies but if they want coverage they have to pay. The one’s who qualify for Medicare and Medicaid will get help from this government program for health care cost and yes some people still pay out of pocket to maintain their health conditions. Technology is one investment to keep up with the growing health care needs of people. We must address these health cost concerns or they will get out of control, so we need to be proactive getting health care financially in shape to accommodate medical service to all. I envision that all people will be covered by health care regardless of economic background. Americans need to get resources to take better care of their health and invest in prevention, so our health will not result in a chronic illness or an incurable disease. Then maybe the law makers in Washington will make sound  decision about our health care plan from other health care providers, so we can benefit and help with cost diminishing. Sports are the basic of everything that a person learns in his/her childhood. There is no known date or time when the sports started. Played from the day the mankind exists. Sometimes played for recreation, sometimes play, sometimes money and sometimes for the pride. There are some people who are not keeping with the sportsmanship in the game and are using some other means to enhance their performance. Sports first played officially In the times of the Romans.When the first time the mall Olympic were held was In the Athens. Now the sports and games came a long way. The standard of the games have increased a lot in the past few years. The competition in the games has increased a lot. The prize money have also increased a lot in the game, from the crown of olive leaves the 1. 6 billion dollars in the past few years. This drastic c hange/elevation in the amount have lead in the increase of the interest in the games as they see a lot of money In these games Firstly considered he waste of time by the people and later became the source of earning money and recreation.Sports are means of physical and mental growth. Sports Increase our capabilities and also improve our efficiency. According to the peoples in the past the sports are just the waste if time which could be utilized in some better ways. As some of the business mans realize that sports is a great way to earn money they invested there money In it. Some of the people think that study is everything and children must be emphasized more on the studies this Is something that I completely disagree with.Everyone got his/her own credibility and we cannot change and we must not change that spouse if Michael Cordon's family asked him to study not to play and didn't provide him the source from the day he was in school. Sports are now a day a great career to be selec ted, as there is a lot of money in it. Sports teaches us the discipline in which the particular sports is to be played and the way in which one must be coordinative with his/her team members, develops In us team spirit. Sports make us mentally and physically more tough and also help us to malignant our body In good heap and make us more active and more stronger.Sports teach the ability of leadership and working under the order of someone. Sports are great way in which one can learn the ability of patience and the ability to accept the defeat. These are some things that most of us find very difficult to learn. As Wifely Shed describes in his writing † the money in the sports is only for the business mans, not for the players† (Willful Shed). The author here want to put emphasis on how the players are exploited by the team owners, as the players are to given the money which they should be given in return of there hard work.As coin has two sides, it is same in the case of s ports and games. There are certain things related with the sports that are not spouse to be part of it. Use of steroids and other performance enhancing products are used very frequently now a days by the players in different sports like bodybuilding, athletics etc. According to news now each and every player in the baseball is on drugs. A-Rod himself in the interview have approved that he was taking some kind of performance enhancing drugs.Some of them are forced to do that but some of them are Just taking them so that they could do better in the games. As the level of competition is raising now the level of usage of steroids is also climbing towards the sky at the same peace. These professional athletes are getting paid a lot of money because they are doing really well in their sports, and why are they doing well? It may be because they have a lot of talent and put a lot of time in training, but also a lot of it could be the use off performance enhancing drugs. So essentially they are getting paid to take the drugs.The athletes that are using the drugs are increasing their talent substantially, and making the honest athletes that aren't taking drugs look bad as they are not getting so many homeruns or get as many quarterback sacks. Now I feel this is not how it should be. The athletes should be tested more frequently, and the ones caught with drugs should be put to a stop as this is spoiling the whole meaning of the sports and fun. Its not only the drug issue but there are certain other issues that are related with the games. Racist discrimination is one of the common things that we encounter in he sports and games.Racism has been a stain on the soul of soccer for generations, and not only in the soccer but also other sports too. If there is a match between a black and a white man the whole community of blacks and the whites think to be the match of there prides. Women's are also criticized in the sports and this thing is becoming common in these days. Women are Just considered good for cheerleaders and some things that they can't be good players but the people forget about the William sisters how they raised the bar by their consistency.Women's are Just thought to be a sort of entertaining thing in the sports, as they look sexy when they are dressed in the cheerleader's dresses. There are always two side of everything this is upon us that which side is worth considering. Sports are very helpful in maintain a good body and moreover it teaches us good morals. Only thing I want to say is that sports are the essential part of our life and we must enjoy it and live that life too. And folks if you want to get rid of this drugs thing than we need to get hard, the players must be checked more frequently to get it done and the against it must be made more strict.

Primary Objective The main objective of the EMU as described in article 105 of the Maastricht Treaty (Jenkins & Economist Intelligence Unit, 1992, p. 466) is the maintenance of price stability. The a rticle goes on to state that â€Å"Without prejudice to the objective of price stability, the ECB shall support the general economic policies in the Community with a view to contributing to the achievement of the Community†. The reasoning for the selection of this objective can be traced to the incentive for the formation of the ECB, which was the fear of rising in inflation due to the dominance of the Germans over the European economic landscape. Hence the EMU has a stated primary objective of keeping the average growth, over the Union countries, of the Harmonised Index of Consumer Prices below two percent (Buti & Sapir, 2002). Monetary Policy In order to pursue this objective the EMU has to choose between the two main macroeconomic approaches. It can either concentrate on an Inflation Targeting approach where a clearly defined objective of numerical indicators of levels of inflation is to be pursued or it can adopt a monetary targeting framework where it expands its resourc es on influencing the monetary aggregate. So far the strategies adopted by the EMU have been described by economists as inclusive of certain aspects of both types of approach; a two-pillar approach. The first pillar in this approach is the money stock manipulation while the second pillar comprises the inflation control strategies. It has been evident for at least a decade that the monetary aggregate indicators do not correspond to the inflation rates which the monetary control purportedly affects (Bofinger, Reischle, & Scha?chter, 2001). Educational technology has become increasingly commonplace in classrooms, and Congress has spent billions to give schools access to technology and online learning opportunities. But research on the effectiveness of using educational technology has lagged behind technologys growth. Certain types of video games can help train kids to follow instructions as well as helping them develop their problem solving and logical thought processes. These skills translate directly to the classroom as students are asked by teachers to complete tasks and are presented problems that require them to use logical problem solving skills. Kids can also learn inductive reasoning and hypothesis testing. Games will often present them with a situation that needs to be solved and this causes the kids to have to develop problem-solving techniques. Before purchasing a video game a parent should talk with a representative at the store to see what the overall premise of the game is to see if it includes elements that will help build these aforementioned qualities. Although there are some positive effects that are possible form video/computer game use there are mostly negative effects. In 2006 45 percent of kids were regular gamers, which means that they played for two or more hours on a daily basis. Performance appraisal also helps in identifying the training needs of the employees. The performance helps the organization provide the proper rewards and recognition to the deserving employees. It also provides the opportunity to the organization for the diagnosis and development. It also helps the organization to better the communication between the employees and the administration. The appraisal system of the organization may have the common practice of measuring the performance of the organization in the form of the numerical or the scaling system. (Armstrong, Michael,2006). The managers are asked to assess the individuals on the scale on the various objectives and attributes. In many companies employees may receive the assessment from their peers, managers, subordinates and customers which is also known as the 360 degree performance appraisal. (Smit, Martin E.J.H.,2006). According to his study American organizations are more focused about individual performance, while Japanese organizations are more focused about group or team performance. 2.2 The Performance Appraisal Process Performance appraisal is planned, developed and implemented through a series of steps, Establish performance standards: Appraisal systems require performance standards, which serve as benchmarks against which performance is measured. To be useful, standards should relate to the desired results of each job. In situations pertaining to embarrassment, performance standards must be clear to both the appraiser and the appraisee. The performance standards or goals must be developed after a thorough analysis of the job. Goals must be written down. They must be measurable within certain time and cost considerations. Communicate the standards: performance appraisal involves at least two parties; the appraiser who does the appraisal and the appraisee whose performance is being evaluated. Both are expected to do certain things. The appraiser should prepare job description clearly; help the appriasee set his goals and targets; analyse results objectively; offer coaching and guidance to the appraisee whenever required and reward good results. The appraisee should be very clear about what he is doing and why he is doing it. For this purpose, performance standards must be communicated to appraisees and their reactions should be noted down right away. If necessary, these standards must be reviesed or modified. As pointed out by De Cenzo and Robbins, too many jobs have vague performance standards and the problem is compounded when these standards are set in isolation and do not involve the employee. Measure actual performance: After the performance standards are set and accepted, the next step is to measure actual performance. This requires the use of dependable performance measures, the ratings used to evaluate performance. Performance measures-to be helpful-must be easy to use, reliable, and report on the critical behaviours that determine performance. Four common sources of information which are generally used by managers regarding how to measure actual performance are personal observation, statistical reports, oral reports and written reports. Performance measures may be objective or subjective. Objective performance measures are indications of job performance that can be verified by others and are usually quantitative. Subjective performance measures are ratings that are based on the personal standards or opinions of those doing the evaluation, and are not verifiable by others. Subjective criteria include ratings by superiors, overall goals, and socio-cultural values of the environment. Compare actual performance with standards and discuss the appraisal: actual performance may be better than expected and sometimes it may go off the track. Whatever be the consequences, there is a way to communicate and discuss the final outcome. The assessment of another persons contribution and ability is not an easy task. It has serious emotional overtones as it affects the self-esteem of the appraisee. Any appraisal based o subjective criteria is likely to be questioned by the appraise and leave him quite dejected and unhappy when the appraisal turns out be negative. Taking corrective action, if necessary: Corrective action is of two types: one puts out the fires immediately, while the other destroys the root of the problem permanently. Immediate action sets things right and get things back on track whereas the basic corrective action gets to the source of deviations and seeks to adjust the difference permanently. This will help him to understand his deficiencies and he will be able to appreciate what the organization is doing for his development. Whatever system suits the organization depends on culture and social conditions of the organization. In times to come, the confidential system will gradually give the way to open system. The need for involving assesses in the process of appraisal can be seen from different angles. One is the need to allow participation by the employee in the process of evaluation. Another need is involving him in setting his goals or targets. Whether the individual should be given an opportunity for self-appraisal as part of reporting process or whether this opportunity should be given to him through an appraisal interview is matter of policy for an organization. What is that opportunity should be given to have a say as to what he himself thinks about his own performance. The chances of getting a good report or bad report are same, as there is no guarantee that the assessment reports would be written on basis of real performance. Often a reporting officer who wants to give a good report about a poor performer would Emphasize the potential aspect of the individual while a very potential may get an adverse report based on his performance. However, a person who is initially connected with day-to-day work of employee concerned should do performance appraisal. This immediate superior should have an important say in evaluating his performance. Self-appraisal should not form the sole basis for any final decision. Management by Objectives Critical Incident Method 5. 360 degrees appraisal Grading Method Forced Choice Method 2.4.1 TRADITIONAL METHODS Graphic Rating Scales method is a printed form which is used to evaluate the performance of the employee. A variety of traits may be used in these types of rating devices the most common being the quantity and quality of work. The rating scales can also be adapted by including traits that the company considers important for effectiveness on the job. From the graphic rating scales excerpts can be obtained about the performance standards of employees. The rating scale method is the most common method of evaluation of an employees performance today. The graphic rating scale may however, suffer from a long standing disadvantage, i.e., it may be arbitrary and the rating may be subjective. In Ranking Method: the ranking of an employee in a work group is done against that of another employee. The relative position of each employee is expressed in terms of his numerical rank. It may also be done by ranking a person on his job performance against another member of the competitive group. The quintessence of this method is that employees are ranked according to their relative levels of performance. While using this method, the evaluator is asked rate employees from highest to lowest on some overall criterion. Though it is relatively easier to rank the best and the worst employees, it is difficult to rank the average employees. Under Paired Comparison Method the appraiser ranks the employees by comparing on employee with all other employees in the group, one at a time. This method results in each employee being given a positive comparison total and a certain percentage of the total positive evaluation. Forced Distribution Method is developed to prevent the raters from rating too high or too low. Under this method, the rater after assigning the points to the Performance of each employee has to distribute his ratings in a pattern to confirm to frequency distribution. Check list Method consists of a list with a number if statements about the worker and his behavior. Each statement of this list is assigned value depending upon its importance. Both statements and their values are derived from preliminary research in which the pooled judgments of persons familiar with the job are used. No restrictions are put on the rater as to the number of statements he should mark. The workers final rating is taken as the average of the scale values of all statements that his superior has checked in rating him. Under Critical Incidents Method, the supervisor continuously records the critical incidents of the employee performance or behavior relating to all characters (both positive negative) in a specially designed notebook. The supervisor rates the performance of his subordinates on the basis of notes taken by him. Grading Method, this method certain well-defined grades are established in advance. Generally; three different grades are used i.e., out standing, satisfactory and unsatisfactory. Employee performance is compared with the defined grades and each employee is allotted a grade that best describes his/her performances. The main limitation of this method is that the rater may rate most of the employers on the higher side of their performance. Forced Choice Method, J.P Guilford developed this method. This method contains a series of statements and the rater rates how effectively a statement describes each Individual being evaluated. The main limitation of this method is that it takes a lot of time and effort to construct several evaluating statements. Under Group Appraisal Method, an employee is appraised by a group of appraisers. This group consists of the immediate supervisor of the employee, other supervisors who have close contact with the employees work, manager or head of the department and consultants. The head of the department or manager may be the chairman of the group and the immediate supervisor may act as the coordinator for the group activities. In Confidential Reports Method, superior appraises the performance of his subordinates based on his observations, judgments and intuitions. The superior keeps his judgment and report confidentially. 2.4.2MODERN METHODS Behaviorally Anchored Rating scales (BARS) method combines elements of the traditional rating scales and critical incidents methods. BARS are a sophisticated method of evaluating employee performance based on employee behavior rather than attitudes or assumptions about motivation or potential. BARS are numerical scale that is anchored by specific narrative example of behaviors that range from very negative to very positive descriptions of performance. BARS are difficult and time-consuming scale to develop. Each job must be analyzed and a list of critical incidents developed by the experts in the job. Once the critical incidents are developed, they are then scaled from effective to ineffective performance. The BARS technique offers high degree of inter rater reliability and objectivity because of its emphasis on behavior. In the Assessment Centre Method, individuals from various departments are brought together to spend two or three days working on an individual or group assignment similar to ones they would be handling when prompted. Observers rank the performance of each and every participant in order of merit. The Human Resource Accounting, deals with cost and contribution of human resources to the organization. Cost of human resources may be taken as standard. Employee performance can be measured in terms of employee contribution to the organization. This technique has developed and still it is in the transitory stage. The concept of Management by Objectives, requires the management to set specific, measurable goals with each employee and then periodically discuss the latters progress towards these goals. Most conceptualiz ations of team performance integrate contextual performance dimensions into formal appraisal. Across the literature, a lack of agreement exists regarding what the specific targets should be in team performance appraisal and how the ratings should be applied. Although the teams effectiveness is the ultimate goal, the performance of individuals and the team as a whole should be evaluated. Levy and Steelman (1997) argue that the traditional supervisor-rated methods of performance appraisal are not appropriate for team settings. Ratings should instead come from all sources in cases where they have adequate opportunity to observe the teams behaviors. Aside from opportunity to observe, the raters relationship with the team may influence his or her perspective; in other words, whether the rater is a team member, team supervisor, customer, etc. Regarding the effectiveness of each rating source, each has advantages and disadvantages. The criteria evaluated by the majority of the literature were inter-rater agreement, agreement with hard criteria, and rating acceptance. From the literature reviewed, the majority of research on rater effectiveness in team settings evaluates rater congruence. This may not even be an appropriate measure of effectiveness, since multi-source appraisal generally proposes that each source affords a unique perspective for evaluating the targets performance. Overall, the effectiveness of multi-source ratings in team performance appraisal has yet to be definitively answered through empirical research. Of the various theoretical models and empirical studies, one common area of agreement is that team performance should be appraised using multiple raters.
